Admission Application

Undergraduate Studies

Graduate Studies

This form is to be used by applicants wishing to apply for admission to any of the University's undergraduate or graduate programs.
Reference Letter Form

This form is to be used for the reference letters required for the graduate studies application.
Transfer of Credits Request Form
(MS-Word)
This form is used to request transfer of credits earned at other institutions.


Registration Forms
Course Enrollment Form

This form is to be used to enroll in courses during the registration period after consultation with your academic advisors.  
Add and Drop Form
(Online) | (MS-Word)
This form is used by learners to add a new course or drop a course which they have enrolled in during the registration period.  
Course Withdrawal
(MS-Word)
This form is to be filled by learners wishing to withdraw from a particular course for which they have been enrolled.
Registration Suspension
(MS-Word)
Learners need to fill this form in order to request temporary suspension of their registration.
Incomplete Grade Form
(MS-Word)
This form is to be filled by learners requiring a delay for submission of a formal assessment after the official end of the semester in which the course was enrolled or in the case where a learner cannot sit a formal end of course examination.
Program Withdrawal Form

This form is to be filled by learners wishing to completely withdraw from a program. Learners need to obtain all signatures as indicated on the form and return it to the Admission and Registration Office within four weeks of the start of the semester.


Evaluation Forms
Course Evaluation Form

This form is to be filled toward the end of each semester to evaluate each course taken.
End of Program Evaluation Form
(MS-Word)
Upon completion of the program learners are asked to complete an online end of program evaluation forms to evaluate the overall program.


Petition Forms
Grade Appeal

Learners can use this form to officially appeal about a grade for which s/he believes an error has occurred.
Complaint Form

Learners may raise complaints expressing their dissatisfaction in any aspect of their program through filling the complaint form and returning it to the LRM office. No complaint will be considered official if it has not been raised by filling the appropriate form
Learner Grievance
(MS-Word)
A learner may pursue a grievance through filling a Learner Grievance Form if s/he believes that a member of the University community has violated his or her rights.


Request Forms
Request for Transcript Form
(PDF)
This form is to be filled to request the issuance of a transcript from the University.
Request for Delay of Payment
(PDF)
Learners may fill this form under exceptional circumstances when they wish to request for permission to defer their course tuitions. All forms for request of delay of payment must be submitted through the LRM Office no longer than two weeks prior to the start of the semester. Under no circumstances will request for delay of payment be processed after this deadline. Learners who did not issue course tuition and did not apply for a request for Delay of payment within the mentioned deadlines will automatically be withdrawn from all courses they have enrolled in.


Important Notes:
In the case where a form is available online, the online form must be filled. The word or PDF file exists only for cases where you may face technical problems while performing the online transactions.

All forms are to be filled in conformance with the University policies and procedures, please ensure you read related policies when filling any of these forms.

Forms are to be filled as per deadlines mentioned in your academic calendar or elsewhere in your program catalog.

Make sure to obtain all required approvals indicated on the form before submitting it to the office of Admission and Registration or to the LRM office.
